The Department of Education (DepEd) issued DepEd Memorandum (DM) No. 004, s. 2022 titled, Implementation of the Results-Based Performance Management System-Philippine Professional Standards for Teachers (RPMS-PPST) for School Year 2021-2022. This memorandum provides guidelines on the performance management and appraisal of teachers using the remaining 18 indicators in the RPMS-PPST as well as the necessary tools, forms, and protocols applicable to all types of learning modalities. Attached below are the RPMS Tools and Forms for Teachers and Master Teachers in all public elementary and secondary schools and community learning centers (CLCs), including those Teachers and Master Teachers who are reassigned as full-time Teacher-Broadcasters in DepEd Central Office (CO).
