The October 2022 Licensure Examination for Teachers (LET) is fast approaching. You might be looking for the PRC Annex A (Informed Consent), Annex B (Health Declaration Checklist), and Annex C (Post Examination Health Surveillance Form) as these forms are part of the things to bring during the examination (Annex A&B). Annex C must be submitted 15 days after the last examination date to the official email address of the Regional Office where you take your licensure examination.
